CITY HALL CLOSURE NOTICE REGARDING RECEIPT OF EMAIL, MAIL, AND ONLINE FORMS

The City business offices will be closed for the holidays beginning at 6:00 p.m. on December 19, 2024, and will resume operations on Monday, January 6, 2025, at 7:00 a.m. Any communications sent to the City via email, mail, or online form submission during this closure will be considered received by the City no earlier than January 6, 2025.

How do I get a copy of a birth certificate or death certificate?

Birth and Death Certificates

Birth and death certificates and records are generally obtained from and maintained by the County where the birth or death took place, in the County Clerk’s Office or its Public Health Office. It is always best to contact the County’s office directly. The below information is provided as a courtesy and does not include all of the potential locations for obtaining records of a birth or death. If you are seeking records outside of San Bernardino and Los Angeles Counties, start by searching your county’s website for the County Clerk’s vital records page. If you are not sure, use the information under the “Statewide” heading below.

Notice: There may be fees involved with these processes. As these processes may change periodically and do not involve the City of Montclair, the best source of information is the County’s office where you would obtain the records. Please contact their offices directly or check their websites to learn more about their processes and fees.


San Bernardino County

San Bernardino County maintains records of births and deaths that occurred within the county.

If the birth or death occurred in the last 24 months…

  • Contact the San Bernardino County Office of Public Health at (800) 782-4264. You may call their office during business hours at (909) 381-8990 if you have questions or need assistance, or visit their website for more information. 

If the birth or death occurred more than 24 months ago…


Los Angeles County

If the birth or death occurred within Los Angeles County, contact the Los Angeles County Registrar-Recorder/County Clerk’s Office at (800) 201-8999 to make request for birth or death records, or visit their website for more information.


Statewide

The California Department of Public Health can provide certified copies of birth or death certificates for those that occurred statewide in certain years. Visit their website or call (916) 558-1784 for more information.
